Pokémon in Pokémon GO have three IV's or "Individual Values":

Attack
Defense
Stamina

Attack and Defense affect CP, and Stamina affects HP. Each IV can have a value up to 15. "Perfect" Pokémon will have 15 in each category. A Pokémon with Perfect IV's will have higher CP and/or HP than one of the same species with lower IV's in any or all of the categories. The difference in CP and HP from the lowest total IV's to highest will vary depending on the species and evolution of the Pokémon.

Some calculators will display your Pokémon against a percentage of "Perfect IV", which is simply the ratio of the IV total for your Pokémon to the total possible IV of 45. A Pokémon with perfect 15's, or a total or 45 IV points, will be a 100% Perfect IV value.

What does perfect IV mean?

Having a perfect IV means the pokemon has a 31 for that stat. When you see someone say a pokemon has 5-6 perfect IV's it means that it has 5-6 stats with IV's as high as they can possibly go for that species of pokemon. Imperfect IV just means it is not a perfect IV, it is less than 31.

How do you know if you have perfect IV?

The team leader will tell you how your Pok\xe9mon ranks using stars and a pretty easy to understand graphic using bars. If your Pok\xe9mon has three stars and a red stamp, it means that it has 100% perfect IVs. If it has three star with an orange stamp, it has around 80-99% perfect IVs.

Perfect IV odds in the wild? There's a one in 4,096 chance, or 0.0244% portability, of finding perfect IVs from a wild catch. There is also a Weather Boosted wild catch though, which gives you a significant odds boost. Instead of the 1 in 4,096 chance, a Weather Boosted catch increases your odds to a 1 in 1,728 chance.
