Why is Diablo 2 laggy when played online with a lot of effects going on (flames)?
Is this a server or a game issue? This happens even on modern PCs. It's when baal-running for example and you got 8 people and a lot of spells going off (especially flames seem to be an issue) it can get laggy and people are afraid the game might be shut down because of dupe-protection. I do not know how this works or why this is. I would really like to know why. I think it's because the game uses old technology that makes it so that with modern hardware you don't necessarily get better performance.

It's a question of the software libraries involved. Direct3D is the graphics card interface library included with DirectX. As you may be aware, DirectX will soon have its 12th version which is supposed to have massive performance improvements.
These optimizations have happened incrementally, so a game like Diablo 2 which utilizes an older version of Direct3D will have poorer performance simply because the library both doesn't work well with modern graphics cards and isn't programmed as well as it is now. Graphics rendering isn't just about having powerful hardware; there have been quite a few bottlenecks that have been cleared up over the years by better drivers.
There is also the question of how well the game itself is programmed to work with libraries like Direct3D or OpenGL, as nonterrorist mentioned.

The whole game lags completely out. I know it was like this in OG d2, but at least in the original, assets would just not be displayed and you could save and exit normally.
