The tooltip for Akarat's Champion - Firestarter states:

Dealing damage burns enemies with the power of Akarat, dealing 460% weapon damage as fire over 3 seconds.

What sources of damage trigger this effect? In particular, I'm asking about the passive equipment procs such as the Fire Chains from Maximus, Fire Walkers, and Pox Faulds. Do those count? What about damage from summoned units (Bowmen from Phalanx) and followers?

Is there a cooldown or do faster ticks/attacks proc the burn more times? Does it stack?

How does the fire starter work?

To use a fire starter, begin by arranging sticks of kindling on top of dried leaves or twigs. Then, hold the magnesium bar over the kindling with the flint strip facing up. Next, drag a knife across the strip to create sparks that will ignite the kindling.

How do you start a fire with a fire starter?

The sharp edge of the flint is used to violently strike the fire steel at an acute angle in order to cleave or shave off small particles of metal. The pyrophoricity of the steel results in the shavings oxidising in the air. The molten, oxidizing sparks ignite the fine tinder.
